
Government funded Community Care Packages are individually planned and coordinated packages of services to help older people with complex needs remain living in their own homes. They are funded by the federal government and delivered by Goodwin, as Community Aged Care Packages (CACP), Extended Aged Care at Home (EACH) and Extended Aged Care at Home – Dementia (EACH D).
Before applying for a package, an Aged Care Assessment Team (ACAT) assessment is conducted to determine the level of need and eligibility.

CACP’s are targeted at frail older people living in the community who require low level support and express a preference to remain living at home. The services provided by a CACP principally meet the daily needs of the recipient through ongoing care or assistance.
Services include personal care (showering, toileting, dressing or undressing), medication, meal preparation, domestic assistance, laundry, shopping or transport to medical appointments.
EACH is intended to assist frail aged people to remain in their homes, who require a higher level of support. Packages are flexible and coordinated to suit the individual’s particular needs and include all services provided by a CACP as well as nursing, in-home respite, and social support.
EACH-D program is designed to provide coordinated and managed packages of care to frail older people, whose dementia is associated with behaviours of concern, who require coordinated, flexible and additional services and supports.
